Why things happen
In organizations, things happen:
- by design - people agreed and implemented something
- by habit - people implemented something without really noticing because that’s what they always do
- by stealth - people implemented something secretly because they were afraid other people wouldn’t like it or the way it was implemented
- by omission - people didn’t deliberately implement anything and the world just changed around them
- by accident - people tried to implement something and got something else without intending to
