Why things happen

In organizations, things happen:

  • by design - people agreed and implemented something
  • by habit - people implemented something without really noticing because that’s what they always do
  • by stealth - people implemented something secretly because they were afraid other people wouldn’t like it or the way it was implemented
  • by omission - people didn’t deliberately implement anything and the world just changed around them
  • by accident - people tried to implement something and got something else without intending to
